SPEAKER REQUEST FORM

Please complete the form below in its entirety. Upon receipt of a completed form, a member of TWI staff will be in touch with you to discuss your request in more detail and approve or deny your request based on its accordance with TWI’s core mission and speaker availability. A standard, daily, honorarium fee will apply for all speaker requests unless an exception is granted. (Note: a reduced honorarium rate may apply for churches and other non-profit entities.)

Who will sponsor the event? Please include a bit of detail about the church/group/organization or provide a web link with information.

What is the vision and purpose of this event/speaking engagement?

Who is the audience for this event?

How large is the audience?

What is the date or dates (including the year) you are requesting?

How many sessions will the speaker need to present and at what time(s) of day (if known)?

How long should the speaker plan to present for each session? Will there be Q & A?

Will the talk be audio or video recorded and, if so, would it be possible for TWI to retain a copy for its own use or to access shared content on your own website?

Are you able to pay an honorarium fee or is this request pro bono?

Where will the event be held and what is the travel time from the airport to the event location?

When would you like the speaker to arrive?

Is it necessary for the speaker to remain at the event after his last session? If so, please provide the earliest time the speaker will be free to fly back to Washington.
